Stroke Nurse Service (NHS Western Isles)

													

The service provides up to date advice, information and support to patients and their families from a registered nurse with specialised knowledge in stroke care.

The service assists patients with a stroke, or Transient Ischaemic Attack (TIA). New strokes are generally offered five follow-up appointments in the first year following discharge from hospital. The Stroke Liaison Nurse will liaise with other healthcare professionals, as well as voluntary services, to help assist patients optimise their full potential.

We offer:

• person-centred care

• to facilitate a smooth transition from hospital to home

• the monitoring of Blood Pressure and offer advice on medications

• liaison with other services.

Clinics can be offered at:

• Western Isles Hospital

• Uist & Barra Hospital

												

Referrals are accepted for patients who have a confirmed diagnosis from their GP, multi-disciplinary team or TIA Clinic.

Patients with a definitive diagnosis of stroke or TIA can also self-refer but their GP will be contacted for additional professional information.
